Sergio,
You're right, the good MUA's don't automatically load images. In that
case you may not get the initial 'open' action if the user doesn't allow
the images to load. In quite a few cases you can get around this with
an embedded iframe but I am not a big fan of using that method because
it has so much potential for abuse. There are also email clients (like
evolution) that will cache the images so they only load once, in that
case you'll never now if the user re-opened the email. As far as
scanning, when I was reading up on email tracking methods I had run
across a site that mentioned some filtering systems will actually load
the images when the email is scanned. I haven't run across this yet
myself and it seems a bit counter intuitive if the scanning mechanism
was trying to detect spam :).
I've also noticed that if you enabled tracking and unsub links in your
email the spam rating will jump at least 1 point with most spam filters
like spamassasin.

I've sent a campaign out to some of our clients and it had a 100% open
rate which to me means that either many users don't have image loading
turned off, or they use one of those crappier clients that offers you no
protection from tracking. So really YMMV because of the way different
MUA's handle images, but so far my experience has been that most users
don't bother to use those MUA's or don't bother to enable the tracking
protections.
