 My guess is that the migration script failed between 6.0 & 6.1 and the
 PBXManager module never got installed/updated properly...

 Check the file in modules/Migration/schema/600_to_610.php for the section
 that deal with the PBXManager migration and see if you can tell if the
 steps have worked and if not where they failed from, then you can try to
 repeat subsequent the steps manually.
