Comment:

 First of all, duplicate Account names are not allowed in 5.0.3.

 Secondly, updating your code for inlcude/utils/listviewutils.php around
 line 798, with the latest code (5.0.4), should solve your problem if in
 case you are enabling duplicate values for Account name.
