On another phpBB board that I'm involved with they use a CAPTCHA for new
user registrations along with a couple extra registration fields. And they
also resort to manual review of suspicious registrations. Have you already
implemented the most popular suggestions from this thread on the phpBB

Another general strategy is to use a third-party filtering service, such as
Akismet or Mollom. Not sure that a mod for integrating Mollom into phpBB
exists, but probably one exists for Akismet. You may need to pay something
for the service, though.
