I guess what I'm asking is how do I construct the php side of it.
custom_field_value[x]
or something like that?
I know how to find it in the database, just not how to referece it in
php.

Look for the associated module -- like in accounts look at the table
accountscf - so if cf_356 was added to accounts custom fields, you
should see a field in that table corresponding...
