Comment (by smythsys):

 Seems to be caused by a duplicate entry (forecast_amount) in the
 vtiger_field table. Deleting one of the records seems to solve the problem
 (I'll confirm). Second duplicate thing I've had to eleminate in that table
 since the upgrade.
