Comment:

 New item can be added to existing menu bar by changing the Menu items in
 Menu editor.
 If you are trying to add a new Menu item under all drop down you can do it
 by creating new
 module using the documentation here,

 the parent property of moduleInstance determines where the new menu option
 will reside under All menu.
