 beside the phone fields under any module that requires a phone number.
 - the phone number field should contain a (+) to the right hand side after
 the phone number input box.
 - upon clicking it, it adds an extra box for an extension
 - this would be used instead of entering (000) 000-0000 x000 in one line.
