 I migrated a couple of CRM from 5.0.4 and have alwaysw the same problem.
 Once I have moved the First Name and the Last Name entry with the modul
 manager to a different location (by accident or by purpose) the Salutation
 field will appear not only in front of the First Name but also in front of
 the Last Name at a contacts or leads detail view.
