 I believe the problem is due to a slight misconception.

 The tag cloud is across the organization. It is not for a specific user.
 It is for a specific module but across the organization.
 So, in your case, I believe the tag might have been already created for
 the Leads module when you happened to try creating a new Lead.

 Please correct me if wrong.
